The fine morning started off with a briefing about the various divisions and functions of the primary health care, activities for the geriatrics age group which included morning workouts and establishments of various clubs which played an active role in geriatrics health.
This is followed by an excursion to the klinik kesihatan Tunjang at around 10.30 a.m. There we were briefed about the green book which is used to record information and track the health record of diabetic patients. This book allows the proper documentation of the health progress of diabetic patients. There are two copies of the green book which are exactly the same in contents, where the larger one is kept as a record by the clinic and the smaller version kept by the patients themselves. The other book was the blue book for recording information of hypertensive patients. There also we able to see the various procedure that done in primary health care.
In the afternoon session at 2.00p.m.
We made a trip to Hospital Jitra where we were briefed on the various documentations needed for tuberculosis patients. These included
1) Patient file record prepared by the Malaysian Ministry of Health Program for National TB Control.
2) TBIS 10 – 1 (Yellow is for pulmonary TB, White is for extrapulmonary TB)
3) TBIS 10 – E (Yellow is for pulmonary TB, White is for extrapulmonary TB
4) TBIS 10C – 3 (Green card) Contact Health Card
5) TBIS 10 A – 1(Information regarding Initiation of treatment for TB patients)$
6) TBIS 10-K (Notification of transfer of treatment centre for TB patient)
7) TBIS 10 – F (Notification of treatment continuation and results for follow up check up)
